BOTOX® (botulinum toxin type A) comes from the bacteria Clostridium botulinum, which produces a toxin that contains 7 distinct serological types. Botulinum Type A is the most potent and is marketed by Allergan as Botox. Type B is marketed by Elan Pharmaceuticals as Myobloc and is FDA approved for cervical dystonia. Safe BOTOX® Cosmetic Injection Practices Prospective patients often wonder why pricing for BOTOX® Cosmetic treatments can vary so widely. While each state has its own specific laws about who is qualified to inject BOTOX® Cosmetic, patient safety is paramount. Any injected medication or product should be performed by a licensed and experienced Physician, Physician Assistant, RN, or Nurse Practioner and ideally under the direct supervision of a physician. In addition, the Centers for Disease Control and Prevention clearly state that vials packaged as single-dose vials should not be administered to multiple patients and leftover contents should not be saved for later use. Julianne Clifford, Ph.D., of the FDA's Division of Vaccines and Related Products Applications, explains "Botox is licensed for marketing and distribution as single-use vials." This means that as packaged, "each vial is intended to be used for a single patient in a single treatment session." BOTOX® Cosmetic does not contain a preservative against potential contamination of the product through repeated use of a single vial. Once opened and diluted, BOTOX® Cosmetic must be used within four hours. Treating multiple people with one vial violates product labeling, which is stated on the package insert, the vial and the carton. So choose your injector wisely. You get what you pay for, and not everyone follows the rules. Risks and complications can arise when untrained personnel administer BOTOX® Cosmetic. How BOTOX® Cosmetic Works The medical term for the action of BOTOX® Cosmetic is selective muscle denervation. Normally, to make a muscle contract, a nerve sends a signal to the muscle. The point where the nerve and the muscle meet is called the neuromuscular junction. When the signal gets to the neuromuscular junction a chemical called acetylcholine is released from the nerve side of the junction and binds to the muscle side of the junction causing more chemical reactions that make the muscle contract. BOTOX® Cosmetic works by blocking the acetylcholine receptors on the muscle side of the junction. Then when the nerve sends a signal to the muscle to contract, acetylcholine is released as before, but it can’t bind anywhere on the muscle. The muscle has no idea it is supposed to contract. It is essentially weakened or paralyzed, but not because it or the nerve have been damaged. This acetylcholine blockade begins within 48 hours. The clinical effects of a BOTOX® Cosmetic injection become noticeable between 5-10 days. Over the next 3-5 months the muscle gradually develops new receptor sites and is able to contract again. The wrinkle smoothing effects of BOTOX® Cosmetic are therefore transient, but maintaining BOTOX® Cosmetic regularly to weaken muscles that cause dynamic wrinkles over time can prevent the formation of static wrinkles, which are creases that become harder to treat.

You Can Always Tell A Woman’s Age By Her Hands The Plastic and Reconstructive Surgery® Journal, the official medical journal of the American Society of Plastic Surgeons (ASPS), published a study in the past discussing the importance of hand rejuvenation. According to the study, most people can accurately tell a person’s age by viewing only their hands: In the study, people examined photographs of female hands and were asked to estimate the women’s ages, i.e., younger than 20 years, 20 to 30 years, 30 to 40 years, etc. In the majority of cases, participants were able to accurately estimate the age of each woman in the photographs. Overall, the physical characteristic which most commonly gave away age was prominent hand veins. Other characteristics associated with older looking hands include thin skin, age spots, wrinkles, deformity, blemishes and prominent joints. Hand Rejuvenation Treatments Two issues need to be addressed for hand rejuvenation. The first is the skin appearance. Brown spots, red spots, broken capillaries, crepey skin, and wrinkles need to be smoothed using resurfacing procedures along with the regular application of skin care products. Secondly, the visibility of the underlying tendons, vessels and joints (skeletonization) needs to be minimized. Injecting fillers under the skin surface hides these structures, while thickening the overlying skin. Prominent veins can be treated with sclerotherapy, lasers, or direct excision. Skin Resurfacing A routine skin care program is an important part of hand rejuvenation. The products that you use regularly on your face can and should be used on your hands. Sunscreen is essential. Light chemical peels of glycolic acid or transchloroacetic acid in low concentration can be used to improve discolorations. Multiple treatments are usually required. Hand skin is different than facial skin, lacking sebaceous glands that are the source of new skin cells. Therefore, hand resurfacing treatments need to be conservative to avoid scarring. Multiple sessions are usually required for desired results. Lasers and other thermal resurfacing treatments such as Fraxel, Carbon Dioxide laser, IPL, Erbium laser, Q-Switched laser, and Thermage can be used, but at lower settings than typically used for the face. So again, multiple treatments are usually required for desired results. Treatments For Skeletonization: Visible Veins, Tendons, And Joints Various injectable fillers can be placed under the skin of the dorsum of the hands to hide vessels and tendons, while thickening the skin. RADIESSE® works well in this area, as it spreads evenly and results are immediate, lasting about 9 months. Injections in this area are relatively painless and there is little to no downtime. RADIESSE® not only adds immediate volume, but it is also stimulatory so it tends to thicken the skin. Hyaluronic acid products such as Juvederm and Restylane can also be used in this area. Sculptra is not recommended in this area. Fat transfer to the hands is another option, but is more invasive with more risk and downtime. Usually the abdomen or thigh is liposuctioned to obtain fat, which is then injected to fill the dorsum of the hands. Because the skin here is very thin, contour irregularities or imperfections in the fatty tissue layer are perceived if it is not precisely performed. Results are very operator dependent. In addition, the results can be unpredictable as a portion of the injected fat can get reabsorbed. Over correction or lumpiness is very difficult to correct.

It is FDA-approved for injection into the mid-to-deep dermis for correction of moderate-to-severe facial wrinkles and folds, such as nasolabial folds. Belotero is a sterile, bioresorbable, non-pyrogenic, viscoelastic, clear, colorless, homogenous gel derived from bacterial fermentation from Streptococcus. Belotero’s chemical structure is different from the other hyaluronic acid fillers on the market. It is a double cross linked hyaluronic gel based on "Cohesive Polydensified Matrix (CPM)" technology that creates a low-viscosity gel product with variable densities of cross-linking in one cohesive structure. This technology creates a thinner substance with uniform particle size that is smooth, easy to inject, and naturally spreads within the treated area when injected. This allows the product to be injected in a more superficial plane without risks of perceptibility or lumpiness. It has a hyaluronic concentration of 22.5 mg/ml. The variable zones of cross-linking allow for the product to integrate nicely into nooks and crannies of the skin and the subcutaneous layer, resulting in smooth, natural-looking augmentation outcomes. What Sets Belotero Apart? Belotero is a soft dermal filler that is able to achieve natural and imperceptible results by integrating evenly into facial skin tissue. Belotero integrates so readily into your own tissue that unlike some of the other fillers, you are unlikely to feel the implant when you touch the treated area. This is especially important in thin skinned areas like your lips and along your lower eyelids, where you will want to see contour improvements without seeing or feeling product. Belotero is a natural hyaluronic acid present normally in the skin as a volumizer and moisturizer. Traditional fillers may enlarge dermal spaces and lift the skin, leaving bumps. The duration of a filler’s effect depends on patient metabolism as well as injection location. Risk And Side Effects The risks and side effect profile are similar to other hyaluronic acid fillers like Restylane and Juvederm. Belotero has a low risk of perceptibility, lumps, Tyndall effect, discoloration, nodules, and irregularity. Patients Who Are NOT Good Candidates For Belotero Patients who are allergic to lidocaine may be candidates, as current formulations of Belotero do not contain lidocaine.
